About this website

This notice describes the Revive Dental Livonia website. It is separate from the practice's notice of privacy practices for patient records and treatment. Contact the office to request information about the handling of your dental records.

Information you choose to share

Our inquiry form asks for your name, email address, an optional phone number, a topic and a message. When the form opens an email draft, the message is sent only when you send it through your email app. When online sending is enabled, the site sends the inquiry to the office through its email provider. We use inquiries to respond to questions and help arrange contact with the practice.

Please use the website for general inquiries. Call the office to discuss medical history, insurance identification numbers or other private health details. The website does not confirm an appointment simply because you send a message.

Technical information

Website hosting systems may process request information such as IP address, requested URL, time, browser details and error status to operate and protect the site. The contact endpoint limits repeated requests. The website does not intentionally put form messages in application logs or browser storage.

External services

Online scheduling, Myla conversations, financing, membership enrollment, maps, videos and review links are provided by separate services. Information you submit to those services is handled under their own privacy notices and terms. The website also requests fonts from Google Fonts and may load a Google Maps embed. The office video connects to YouTube only when you choose to load it or follow the link. Your browser sends connection information to those providers when their content loads.

Cookies and tracking

This version of the website does not include advertising pixels or third-party analytics. It may use a short-lived cookie after a successful inquiry to display the correct confirmation screen. External services can use their own cookies. See the cookie information for details.
